Many residents we support are at a transition point: discharge from hospital, loss of previous accommodation, or a change in mobility or health that makes their existing housing unsuitable.
What we provide
Safe, accessible accommodation matched to physical and mobility needs.
Housing management with tenancy sustainment and move-on pathways.
Coordinated work alongside registered care providers, health services and local authority teams.
Early identification of risk and proactive support to maintain placement stability.
Clear outcomes, reporting and oversight for commissioning partners.
How we work
We operate a partnership model. Placements come via commissioners and professional referrers, never direct to the public. This keeps standards high, risk well-managed, and communication clear across agencies.
